Valve adjustment went sideways on an old 3406E in Lubbock

I was doing a routine valve lash check on a 3406E at a fleet yard in Lubbock last Tuesday and the pushrod bent clean in half when I torqued the rocker arm bolt. Turns out the previous guy had left the jake brake lobe in the wrong position, so my feeler gauge read fine but the clearance was actually off. Anyone else run into hidden damage like that after a simple adjustment job on these older Cats?
