Running a job on a 5-axis Mazak down in Austin last Tuesday, and I left a 3/8 carbide end mill sticking out way too far. Chip load was fine on paper but the tool snapped on the first full pass through 4140 steel and took out a vacuum fixture with it. Replaced the end mill and the fixture plate, about $300 total, plus a day of re-setup. Anyone else had a cheap mistake like this turn into a bigger bill than you expected?
