Caught a defective weld seam with my phone flashlight at a refinery shutdown in Gary

We were doing a tie-in on a 12-inch line and the inspector was running late, so I took a closer look with my phone's light and saw a hairline crack running through the root pass. Turns out the previous shift had skipped preheat because the heater was on the fritz, and the crack only showed up once the pipe cooled down. Has anyone else caught stuff on a night shift that the regular inspection missed just because you slowed down for a minute?
