Why does nobody talk about telling a coworker their breath smells

Tbh I had a guy at the practice last Tuesday who was leaning right over me while I was cleaning his wife's teeth and his breath was so bad I almost gagged. I wanted to say 'dude you need a tongue scraper' but I just smiled and handed him a mint instead. Has anyone else had a moment where you wanted to give someone honest hygiene advice but knew it would wreck the whole conversation?

Friend told me she once breath-sprayed a coworker's coffee cup by "accident" after their third morning meeting in a row. Guy never noticed but his breath was minty fresh for weeks.

Does it make me a horrible person that I would've rather had that conversation than the time I accidentally told a guy his fly was down but it was actually just his shirt tail? I swear I felt his soul leave his body. Handing over a mint is a solid move though, I've done that exact thing with a patient's husband last month when he kept talking right in my face about his golf game.
